I definitely wouldn't say zero effort, personally if the writing was actually good I'd be fine to give the visuals a pass (except the ridiculous penis/pube sizes, lol).
It's just the lack of writing skill makes it feel kind of sterile, which (for me) then draws a lot more attention to the fact that he's financially dependent on it being long, rather than whatever's necessarily supposed to be happening in the story itself

I think you're misinterpreting what you're seeing in panel four. It's not that Jacob's hand is coming out of his thigh directly.

What you're actually seeing is two separate camera angles within the same panel. The left side of the panel is, more or less, Jacob's point of view as he's fingering Clara. The right side of the panel is a shot of Jacob's hips and dick with the "camera" looking at him from his left.
